肺AAH、AIS和MIA，分別是什么意思？

肺癌從癌前病變，發(fā)展到早期肺癌的三個(gè)階段。

1是AAH，非典型腺瘤樣增生，屬于癌前病變，還不是癌。

2是AIS，原位癌，剛剛發(fā)生癌變，很局限，尚未出圈來搞破壞干壞事，屬于最早期，0期。

3是MⅠA，微浸潤癌，突破了界限，出來攻城占地了，真正的肺癌，肺癌分期中的最早期，I期中的Ia期。

AAH階段不必要手術(shù)。AIS、MⅠA為最早期肺癌，完全切除或局部切除即可達(dá)幾乎100%的治愈率。

貴在早期發(fā)現(xiàn)，早期肺癌手術(shù)切除可治愈。

AAH：atypical adenomatous hyperplasia，不典型腺瘤樣增生；AIS：adenocarcinoma in situ, 原位腺癌；MIA：minimally invasive adenocarcinoma，微侵潤腺癌。是新的肺腺癌病理學(xué)分類名詞。

2011年2月，國際肺癌研究協(xié)會(huì)(IASLC)、美國胸科協(xié)會(huì)(ATS)及歐洲呼吸學(xué)會(huì)(ERS)在《Journal of thoracic oncology》上聯(lián)合發(fā)布了肺腺癌國際多學(xué)科分類新標(biāo)準(zhǔn)，即新版肺腺癌 IASLC/ATS/ERS分類：

(1) 不典型腺瘤樣增生(atypical adenomatous hyperplasia, AAH)：肺末梢組織局灶性增生，≤5mm，不典型的立方或柱狀上皮細(xì)胞代替原來的正常肺泡上皮細(xì)胞，沿肺泡或呼吸細(xì)支氣管分布，是AIS的癌前病變。

(2)原位腺癌(adenocarcinoma in situ, AIS)：≤3cm，原版細(xì)支氣管肺泡癌(bronchiolo-alveolar carcinoma, BAC)，分非粘液型、粘液型、混合型。

2. 微浸潤腺癌(minimally invasive adenocarcinoma, MIA)：

≤3cm，鱗屑生長(zhǎng)方式，浸潤≤5mm，分非粘液型、粘液型、混合型。

3. 浸潤性腺癌(invasive adenocarcinoma, IAC)：鱗屑樣為主型(lepidic predominant, LPA，為IAC 主要類型，鱗屑生長(zhǎng)方式，浸潤>5mm，原版非粘液型BAC)、腺泡為主型、乳頭為主型、微乳頭為主型、伴粘液的實(shí)體為主型(以實(shí)性生長(zhǎng)方式為主，伴粘蛋白產(chǎn)生)。

4. 浸潤性腺癌變異型(invasive adenocarcinoma variation, IAV)：浸潤性粘液型腺癌(invassive mucous adenocarcinoma, IMA，原版粘液型BAC)，分膠質(zhì)型、胚胎型、腸型。
